Thank you for serving as a Group Leader!  These groups would not happen without you.  I really appreciate your commitment each week.  Since these groups are new, we still have a lot to learn.  This survey will help me see what is working and what is not working.  Please be completely honest as you answer these questions.  We will not be able to grow and improve without your constructive feedback.
